Graduate Student Forms

M.A. STUDENTS

  • Advisory Committee Form
    You must submit an Advisory Committee Form, typically during your first year.  
  • Program of Study
    MA students must submit a Program of Study Form for approval by the department's Graduate Coordinator and the Graduate School before submitting your thesis. This form is available on the GradStatus website under Forms.
  • Format Check
    When preparing to defend your thesis, you must submit one complete copy of your thesis for a format check with the Graduate School. 
  • Approval Form for Master’s Thesis and Final Oral Examination 
    This form must be "ready" before you take your final oral examination, that is, before you meet with your advisory committee for your thesis defense. Therefore, students must submit the form before their defense date. After the defense, your advisory committee will receive the form and will record the result of your defense on the form. Click here to initiate this form through GradStatus before the date of your thesis defense.
  • Electronic Thesis and Dissertation (ETD) Submission Approval Form
    After your final, corrected thesis has been fully approved by your advisory committee, you must then submit (1) the final ETD Approval Form and (2) a final, corrected copy of your electronic thesis/dissertation that adheres to the required Graduate School format. 

PH.D. STUDENTS

  • Advisory Committee Form
    You must submit an Advisory Committee Form, typically during your first year.  
  • Program of Study
    PhD students must submit a Program of Study Form for approval by the department's Graduate Coordinator and the Graduate School by the time the notification of the preliminary oral comprehensive examination is given. You have to make sure this form is approved before your oral comprehensive examination is scheduled. This form is available on the GradStatus website under Forms.
  • Comprehensive Exam Announcement
    This form must be filed by the Graduate Coordinator. After completion of the written comprehensive examination, the oral examination is announced to the Graduate School and posted on the Graduate School website. The Graduate Coordinator will file this form through the Grad Status website.
  • PhD Proposal/Prospectus Form 
    PhD students in Religion must meet with their full committee for a Prospectus defense, after which the Prospectus form is signed and submitted to the department's Graduate Coordinator. 
  • Application for Admission to Candidacy
    You can find the requirements for PhD Admission to Candidacy here. Once you meet the requirements, you can submit the application for admission to candidacy. This form must be signed by your Major Professor and the Graduate Coordinator and filed with the Graduate School. You need to initiate this form through GradStatus.
  • Format Check
    When preparing to defend your doctoral dissertation, you must submit one complete copy of your dissertation for a format check with the Graduate School. 
  • Dissertation Defense Announcement
    This form must be filed by the Graduate Coordinator at least two weeks before the date of your defense. The Graduate Coordinator will file this form through the Grad Status website.
  • Approval Form for Doctoral Dissertation and Final Oral Examination
    This form must be "ready" before you take your final oral examination, that is, before you meet with your advisory committee for your dissertation defense. Therefore, students must submit the form before their defense date. After the defense, your advisory committee will receive the form and will record the result of your defense on the form. Click here to initiate this form through GradStatus before the date of your dissertation defense.
